  This form is for corporate policy owners, policy owners with a collateral assigned policy who are not POSB or DBS account holders, and for policy owners who wish to set up a GIRO arrangement with banks other than the nine participating banks.
  
  If you would like to set up a GIRO arrangement with any of the nine participating banks - DBS/POSB, OCBC, UOB, Citibank, Standard Chartered, HSBC, Maybank, ICBC and Bank of China - please apply online.
