Revoke a beneficiary from a revocable nomination

Revoke an existing revocable nomination and make a new nomination.


What you need to know

Under Section 49M of the Insurance Act, a policyowner may revoke an existing revocable nomination and make a new nomination at any time.

Instructions

  1. Download the Revocation of Revocable Nomination Form and ensure all mandatory fields are filled out. 
  2. Print out the form and obtain wet-ink signatures from both yourself and your two signatories (i.e.. witnesses).
  3. Mail the completed original hardcopy form to us via the following address:

The policyowner and two signatories (witnesses) must sign the form.

  • All witnesses must be at least 21 years old.
  • A witness must not be the named beneficiary/nominee or the spouse of a beneficiary/nominee. 

The processing time for your request is 4 business days from the date we receive your submission for standard cases.

Please allow additional time for us to process non-standard cases. We will update you in writing regarding any outstanding requirements or the status of your request.
