Revoke a beneficiary from a trust nomination

Instructions

Under Section 49L of the Insurance Act, you can revoke a trust nomination if all nominees who are at least 18 years old have given consent. If any of your named nominees is below 18 years old, you will need to obtain written consent from a parent or legal guardian who is not the Policyowner. Alternatively, you can obtain consent from a trustee who is not the Policyowner. 

To revoke a trust nomination, please complete the Revocation of Trust Nomination Form and mail it back to us.

Who will need to sign the form to activate the change? 
For a Trust policy under S49L, the Policyowner and all Trustees who are not the Policyowner must sign the form.
